After attaching icon filename to menu items i am getting the following error while running the forms. See screenshot below. When the icon file name in the corresponding menu items are removed this error is not happening. I am working on Forms 10g
Any one have idea why this happens ?

For documentation on deploying icons, go to http://www.oracle.com/forms/10g/help/ then 'Deploying Forms on the Web', then 'Deploying Icons', where it refers to 'UI_ICON' (sorry, my bad memory for saying 'TK_ICON').

To deploy Forms applications on the web that contain icons in buttons and in tree control items, you'll first need to convert the icon files into GIF (GIF 89a ) or JPG format.
Unlike the client/server model of previous versions of Forms, in Oracle Forms there is no UI_ICON variable to locate icons used in Forms applications. Instead, image icons in Oracle Application Server Forms Services are referenced by a URL that is constructed internally based on three things: the "imagebase" parameter in the formsweb.cfg configuration file, the icon name in the items property palette, and the default icon path setting in the Forms Registry.dat properties file.
At runtime, icons in your Forms applications are referenced through a Java Applet.

I find this text a bit contradictory. You need the UI_ICON path so that the Form Builder and Developer Server knows where to find the 'gif's on your PC (or network). The Application Server uses other files.
